Responsibilities:

The state-wide better at Home initiative has been implemented to deliver more healthcare within the home. Part of this initiative is to enable patients to access Hospital in the Home (HITH) and District Nursing Services.
South West Healthcare (SWH) is seeking a motivated and skilled Registered Nurse to join the Hospital in the Home (HITH) team. As a valued member of our multidisciplinary team, the Hospital in the Home Program Coordinator will be responsible for the ongoing development of the program and provision of clinical care throughout our community.
Our teams work using systematic mechanisms that ensure regular communication and the flow of information with other clinical staff and professionals, families, carers and stakeholder groups located both internally and externally to SWH.
