About the role
This is an exciting time to join our rough sleepers’ team! Following the creation of West Northants Council in 2021, we are on a journey of shaping what the new rough sleepers service looks like. What will you be doing?
To provide a high quality, outcome-focused support to rough sleepers who have complex needs and require a co-ordinated, multi-disciplinary approach to their housing, health, and wellbeing. To work collaboratively and imaginatively with other services to develop and implement multi-agency support to recovery, and resettlement.
To help and prevent delay in Hospital discharges and assisting to housing solutions to enable patience to be discharged into accommodation, that suits their needs, ensuring effective coordination and communication with housing teams and partnerships.
About you
You don’t need experience across all aspects of the job as that will form part of your development journey, but you will need to be outcome- focused and experience working with complex needs and require a co-ordinated, multi-disciplinary approach to their housing, health, and wellbeing.
To build relationships and knowledge across the housing and health sectors to help support people to remain in their own home, in particular to work in partnership with partners in the health sector to prevent homelessness and promote timely and safe discharge into suitable accommodation among patients leaving acute and community hospitals.
You’ll need to have a solution driven and adaptable approach to working, thrive in a busy environment with a positive approach to challenges! Ideally you will have previous experience of working with multi agency services with experience of working with people with behavioural, physical, social and welfare needs.
