Description:
We are currently looking to hire a Housing Coordinator - Extra Care to join Torus! The Housing Coordinator will play a vital role in assisting and empowering residents to maintain independent living.
This is a permanent post based at Harpers Green in Warrington, one of our purpose-built extra care schemes commissioned by the local authority to deliver 24/7 care and support to people aged 55+.
The facility has a dedicated Torus housing team who work collaboratively with the care provider to ensure all residents are able to access opportunities to actively engaged in social activities and events on their schemes and in their local communities. The scheme includes commercially operated bistro and salon which offers an exciting opportunity for a local business to deliver services to residents and people in the surrounding areas.
SKILLS AND EXPERIENCE:
- Experience of working with older and or vulnerable people within a housing or social care environment.
- Understand the social, physical and mental health needs of an ageing population and the ability to promote their general wellbeing and quality of life.
- Track record of completing assessments and developing support plans with targets and outcomes for clients.
- Experience of managing a case load and multi-agency working.
- Experience of crisis management, risk minimisation and of making decisions in difficult situations
- Manage your personal resilience when dealing with difficult situations and identifying mechanisms to deal with these.
- Have knowledge and experience of Safeguarding Practices and Procedures.
- Be committed to ED&I in the workplace and in the delivery of services to customers in the scheme.
- Competent with a range of ICT software packages and systems.
Responsibilities:
- Carry out viewings of properties with applicants and potential applicants, promoting the benefits of extra care services.
- Develop personal plans and agree interventions that enable residents to manage their tenancies safely and successfully.
- Assist residents with settling in and maintaining their tenancy, including understanding their rights and responsibilities as residents.
- Assist residents in accessing a full range of universal and preventative services.
- Make the appropriate referrals and demonstrate positive multi-agency working with the relevant agencies when there is a change in the resident’s circumstances, where any unmet need is impacting on the person’s quality of life and capacity for independent living.
- Work collaboratively with other business areas, including Allocation and Voids Teams, to complete sign-ups of new tenants and ensure minimum void rent loss.
- Arrange for adaptations and the installation of assistive technology when required by residents to maintain their independence and quality of life.
- Carry out needs and risk assessments to identify needs, strengths, and aspirations in relation to independent living skills.
- Link support plans with care plans where residents are receiving care services and ensure relevant external agencies are involved where appropriate.
- As a minimum, monitor and review personal plans either every 6 months, as and when there is a change to the resident’s circumstances, or if the resident asks for a review.
- Monitor the health and wellbeing of residents through effective support planning, responding as appropriate to any changes.
- Ensure full family involvement in the planning of care and support in line with individual resident wishes.
- Ensure services are flexible, responsive, and personal to the needs and preferences of residents. Ensure that this is reflected in individual support plans and information is communicated where appropriate to other support workers.
- Ensure residents are offered opportunities to maintain and develop new skills by participating in a range of stimulating health and wellbeing activities both on the scheme and in the wider community.
- Facilitate links between residents and the wider community, including activities which enable people to continue to make a positive contribution to their community and which reduce social isolation.
- Support and engage residents in ways that promote and maximize independence, choice, and health & wellbeing.
- Work collaboratively with the team to ensure data is collated, accurate, and inputted effectively into the system to support the management of scheme performance.
- Contribute to scheme audits and the completion of any action required.
- Take responsibility for own performance, sourcing guidance and support where required.
- Work flexibly in line with the needs of the service. Occasional evening and weekend work may be required. Flexible to work across all other Extra Care Schemes when required.
