POSITION SUMMARY
The Housing Navigator (HN) is responsible to engage eligible members in housing transitions, and tenancy navigation services designed to significantly enhance member’s quality of life through innovative health and support solutions. These solutions are provided to improve health outcomes, manage healthcare costs, and foster independence. The Housing Navigator is a member of a team dedicated to providing high-quality care to Managed Care Plan Members in a compassionate, effective, timely, and professional manner.
EDUCATION AND EXPERIENCE
- Bachelor’s or master’s degree in related field or commensurate experience in the housing/community services field.
- One or more years’ experience in managed healthcare and/or community-based case management. Experience with unhoused populations and chronic medical or mental Illness preferred.
- Familiarity with the California Advancing and Innovating Medi-Cal (CalAIM) and the managed care environment.
REQUIRED SKILLS
- Proficient with Microsoft Office suite.
- Strong verbal and written communication skills.
- Ability to work well with others, including staff, clients, members, and the public.
- Maintain all required licensure, certifications, and vaccinations (including proof of up-to-date Covid and Influenza vaccinations). Exceptions may apply but all exemption requests must be approved by Human Resources before employment.

Responsibilities:
- Provides comprehensive housing transitions and housing tenancy navigation support to members eligible for services.
- Establishes and maintains effective communication with members, building and strengthening relationships. Fosters collaborative relationships with providers, facilities, community resources, etc.
- Conducts telephonic and in-person outreach and engagement activities to unhoused and housing at-risk members, who are identified as eligible for services.
- Completes screenings and assessments to identify housing needs and factors contributing to housing instability, including functional, environmental, employment, educational, and health information, as appropriate.4.Identifies and facilitates access to suitable housing options for members, as well as application processes, move-in activities, rental negotiations and service referrals. Conduct facility and habitability inspections, as needed.
- Facilitates access to supportive services such as job training, life skills development, legal services financial resources and health care providers, in collaboration with Care Managers. Provide crisis intervention, as needed.
- Fosters working relationships with property management firms, property owners, housing services agencies and housing advocacy organizations. Engage with landlords and familiarize them with the ILS-CA’s care management and housing services and community resources to assist in preventing and addressing tenant issues.
- Effectively manages caseload, complying with policies and procedures, workflows, documentation requirements, and timelines. Document all interventions and required information in eCare, Health Management Information Systems (HMIS) and other designated databases, in an accurate and timely manner and according to guidelines. Respond appropriately safety issues, emergencies, suspected abuse/neglect and other critical situations.
- Completes administrative responsibilities: read/respond to emails and eCare tasks; complete timecards accurately, and appropriately track and submit mileage reimbursement requests.
- Attends staff development sessions, workshops, and conferences, as requested.
- Performs other duties as required or assigned.
