The Mustard Seed is committed to helping people who are homeless transition toward and maintain sustainable and healthy lifestyles. The Housing Support Worker Team Lead is a member of a team that provides housing and facilitates support services for low to mid acuity persons experiencing multiple barriers, who require the support of Permanent Supportive Housing (PSH).
The role of the Housing Support Worker Team Lead will be to provide case management services, assist clients with daily living skills, connecting to community resources and other day to day tasks as needed. The Housing Support Team Lead will work with the Housing team to provide ongoing addiction and mental health support. The Housing Support Team Lead will be working onsite at the residence which functions within a recovery reduction model.

Option 1: Monday to Friday, 7:00am-3:30pm
Option 2: Monday to Friday, 10:45am to 7:15pm

MINIMUM QUALIFICATIONS AND SKILLS

  • Degree/Diploma in Social Work or related Human Services field
  • Two years experience working in Social Work or related Human Services field
  • One year experience working with those experiencing homelessness; direct experience with Housing First initiatives considered a strong asset
  • A passion to be part of ending homelessness in Saskatchewan
  • Knowledge of Saskatoons’ community resources
  • Experience with case management is considered a strong asset
  • Experience working with mental health and/or addictions is a strong asset
  • Demonstrated competency and ability to practice crisis intervention
  • Ability to de-escalate and problem solve with individuals under the influence and/or struggling with a mental health crisis
  • Ability to establish and maintain rapport with culturally diverse clients, including a familiarity with the Indigenous culture
  • Ability to adhere to confidentiality standards and FOIP Act stipulations
  • Excellent written, oral and interpersonal skills with the ability to effectively communicate with clients, members of a team, and community agencies
  • Excellent organizational and time management skills
  • A self-starter with the ability to work in a cross-functional team
  • Ability to accomplish goals with minimal supervision
  • Cultural sensitivity and respect for client individuality and self determination
  • A broad knowledge of and experience working with Indigenous communities is an asset
  • Proficient computer skills (Microsoft Office Programs); experience with Efforts to Outcomes (ETO) is an asset

ADDITIONAL REQUIREMENTS

  • As The Mustard Seed is a Christian ministry, we require our employees sign an agreement indicating that they will respect and not detract from our Statement of Faith.
  • Must have legal authorization to work in Canada on a full-time basis for anyone other than current employer
  • A satisfactory employment background check and vulnerable sector search, is required
  • Must be able to drive and meet all TMS requirements to do so
    This position requires the use of a company vehicle. Candidate must have and maintain valid driver’s license and will need to consent to a driver’s abstract request.

Responsibilities:

  • Collaborate with the Housing Team to create and implement workflow procedures and innovative programs
  • Review, coach and support the housing team with case management and resident documentation
  • Assess client’s level of intoxication, risk factors, immediate needs and vulnerabilities and then inform supervisor of any concerns
  • Meet with clients housed in The Mustard Seed apartment units on an on-going weekly basis
  • Provide ongoing client support with daily living skills, maintaining residency. Includes assisting clients with achieving their recovery goals.
  • Document client interactions, service plans and client progress in delegated software
  • Maintain working knowledge of relevant community supports and resources
  • Function as an advocate to assist clients with accessing community resources and external supports
  • Provide crisis intervention, as necessary
  • Provide input into program development and community resources
  • Attend scheduled team meetings and training sessions
  • Develop and maintain effective working relationships within The Mustard Seed and with external agencies
  • Active role in community programs and in house activities
  • Attend weekly team meetings and other training as required
  • Complete required training including but not limited to FOIP, First aid and CPR, Naloxone, NVCI and De-escalation certification and attend meetings
  • Provide support with finding secure housing for residents
  • Provide medication assistance to residents who use the medication assistance program and coordinate with pharmacies as needed
  • Maintain professionalism and adhere to all policies and procedures including but not limited to Health & Safety and HR policies and procedures
  • Other duties as assigned by Supervisor
