Description:

Within the context of LSC policies and procedures, Medicaid standards, governmental contracts, state and federal guidelines, and program service manual procedures, IAFT Care Coordinator provides clinical guidance, behavioral intervention and supervision to youth and families in the IAFT program. The IAFT Care Coordinator works intensely with the IAFT team by providing ongoing support that serves to stabilize the placement and facilitate behavioral change necessary for successful transitions for severely emotionally disturbed youth in care. This includes facilitating the setting of placement goals and desired outcomes established by the placement team. The IAFT Care Coordinator is responsible for writing and monitoring behavioral intervention treatment plans, monitoring progress according to the level of care, and documentation and planning transitions for discharges.
Essential Functions
1. Follow all of the policies and procedures established by Lutheran Services Carolinas.
2. Responsible for identifying, linking and coordinating initial and ongoing systems of care and wrap services based on assessment activities.
3. Assist in facilitating placement matches. Consult with team members, assess appropriate resources for youth and IAFT families and assist with matching. Assist with obtaining all placement paperwork, which includes clinical information and contractual documentation (financial agreements and medical necessity documentation).
4. Attends daily and weekly clinical team meetings and regularly updates team of progress to gather information necessary to deliver the clinical interventions for the week.
5. Delivers and performs clinical interventions as directed to child and family as instructed by the clinical team.
6. Responsible for writing and monitoring behavioral intervention treatment plans, monitoring progress according to the level of care, documentation and planning transitions for discharges.
7. Maintain documentation in the youth’s file and IAFT parent file according to state and federal standards. Complete documentation for all services provided to each individual for both billable and non-billable services.
8. Utilizes the database to input relevant required data in as required.
9. Complete all placement and attendance forms as required to facilitate billing and ensure all authorizations remain current.
10. Provide ongoing support and supervision to IAFT parents who carry out therapeutic interventions for the youth in care. Assist with crisis interventions by diiffusing the current crisis, evaluating its nature, and intervening to reduce the likelihood of a reoccurrence.
11. Assist in marketing Lutheran Services Carolinas programs with potential referral sources. Have the knowledge and ability to respond independently and promptly to marketing and community relations needs, and work in a self directed manner so that all other work obligations are met according to the requirements of this position.
12. As a member of a regional therapeutic clinical team, the IAFT Care Coordinator is empowered under the guidance of the Regional Team Manager to manage regional team operations, which includes facilitating placement goals, recruitment and licensing, budgeting, and promoting LSC within the child welfare community.
13. Serve on committees and projects at Lutheran Services Carolinas.
14. Utilizes Person thinking planning model and assists with the creation of the Person centered plan.
15. Respond to other duties as requested.
Education: Bachelor’s degree in a field related to social work and two years of related experience, two of which must be in child welfare; or a Bachelors degree in a non-related field and 5 years of related experience, at least two of which must be in the child welfare; or a Master’s degree in a human services related field and one year of experience in child welfare.
Experience: Must meet the qualifications listed above.
Specific skills/abilities: Must have specific knowledge of child welfare systems. Must have skills in group and team facilitation, completing assessments and writing behavioral treatment plans. Must be self-motivated, have good time management skills and have the ability to work independently. Must be flexible and be able to work irregular hours.
Specialized knowledge, licenses, etc: Must be at least 21 years of age, possess a valid driver’s license and have reliable transportation. Must have knowledge of family systems, childhood developmental and attachment theory, psychopathology, behavior management, psychotropic medications
