Description:
Case management is a recognised national role within the NHS and a national network of peers is well established who work cooperatively to manage complex patient cases. Case managers come from all professional backgrounds and disciplines, which adds to the strength of these teams, and as such we would like to hear from candidates from any professional discipline who consider they fit the criteria for this post. Possession of a relevant professional qualification is essential, as well as knowledge of wider mental health services and ICS systems. Direct experience of working in specialised secure care mental health, learning disability & autism and forensic services is desirable.
In addition, an understanding and experience of dealing with quality and clinical governance systems is required, as well as a proven track record of having excellent interpersonal and organisational skills. Full induction, training and opportunities for continuous professional development will be provided. We have access within our collaborative to nationally respected clinicians who are available to support with clinical supervision, professional development and mentorship expected for this role. While there will be home working, travel within the East Midlands and beyond will be expected for this post.
