Description:
Our community services are core to the delivery of the ICPs strategic aims, working to improve our populations health, supporting them when they are unwell, and achieving fairer outcomes for both children and adults across our system.A core pillar of enabling delivery of the ICS Strategic aims is the transformation of Community Services. The ICS recently published its delivery plan for Primary and Community Care which builds on our existing strategies and national policy and guidance.As an ICB we have started an ambitious community services procurement through the Integrated Community Based Care Programme with the aim that this will allow us to accelerate transformation and unblock the significant opportunities that exist to deliver the left shift of care and improve outcomes for our communities. To support both the immediate work needed to deliver improvement across Community care at pace, and to achieve a successful transition to the new contract from 2025, we are creating a new Community Services and Planned Care team which will be hosted alongside the UEC team.This new team will have responsibility for all aspects of Community Care on behalf of the ICB.The team builds on the expertise across the ICB to create a single and dedicated function.1.JOB SUMMARY & KEY RESPONSIBILITIESThe main purpose of the role is to directly manage and deliver the work of the Community functions and, responsible for day to day work assigned to Project Team and to support the development and delivery of the Integrated Community based care programme (ICBC). Improvement Manager (LTC) 8aThe Post holder will be responsible for delivering the Community functions including: Support the delivery of the Primary and Community Delivery Plan providing programme and project support to the Primary and Community Delivery Group and associated sub-groupsProvide Programme support to the ICBC Programme, including delivery of the strategic transformation priorities and outcomesWork with the Place team and other Portfolio teams to ensure delivery of the ICP strategic aims, ensuring effective use of the collective capacity and skills and the ICB teams.Work with providers and partners to drive forward service improvement informed by health insights and intelligence, paying particular attention to addressing variation and inequalitiesDeveloping subject matter expertise and leadership capability within the ICB, with a view that this team over time will also work in support of the development of the Community Provider Collaborative in BSWWork with other teams within the ICB to ensure the delivery of the ICP strategic aims.
In particular this team will work closely alongside the Place Team and Primary Care Team to support planning and delivery.The role supports the business in driving transformation as well as value for money in planning, commissioning and service.The role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.The job description and person specification are an outline of the tasks, responsibilities and outcomes required of the role.
