Description:
An exciting opportunity has arisen here at BSW ICB for a 12 month fixed term contract (Maternity cover) for the role of Improvement Manager – Long Term Conditions.
This will provide fantastic development for an individual to lead on the transformation and delivery of several work streams within our community services as well as support the implementation of the new ICBC contract in collaboration with the programme team.
The Post holder will be responsible for delivering the Community functions including:
- Support the delivery of the Primary and Community Delivery Plan providing programme and project support to the Primary and Community Delivery Group and associated sub-groups
- Support to the ICBC Programme team in the delivery of the
strategic transformation priorities and outcomes
- Work with the Place team and other Portfolio teams to ensure delivery of the ICP strategic aims, ensuring effective use of the collective capacity and skills and the ICB teams.
- Work with providers and partners to drive forward service improvement informed by health insights and intelligence, paying particular attention to addressing variation and inequalities
- Developing subject matter expertise and leadership capability within the ICB, with a view that this team over time will also work in support of the development of the Community Provider Collaborative in BSW

BSW ICB have adopted agile working so this means the successful applicant will predominately be working from home but may be requested to work in any one of the ICB’s office bases across BaNES, Swindon and Wiltshire. You will also have a contractual base within the BSW ICB office where your team is based.
Responsibilities:
JOB SUMMARY & KEY RESPONSIBILITIES
The main purpose of the role is to directly manage and deliver the work of the Community functions and, responsible for day to day work assigned to Project Team and to support the development and delivery of the Integrated Community based care programme (ICBC). Improvement Manager (LTC) 8a
The Post holder will be responsible for delivering the Community functions including:
Support the delivery of the Primary and Community Delivery Plan providing programme and project support to the Primary and Community Delivery Group and associated sub-groups
Provide Programme support to the ICBC Programme, including delivery of the strategic transformation priorities and outcomes
Work with the Place team and other Portfolio teams to ensure delivery of the ICP strategic aims, ensuring effective use of the collective capacity and skills and the ICB teams.
Work with providers and partners to drive forward service improvement informed by health insights and intelligence, paying particular attention to addressing variation and inequalities
Developing subject matter expertise and leadership capability within the ICB, with a view that this team over time will also work in support of the development of the Community Provider Collaborative in BSW
Work with other teams within the ICB to ensure the delivery of the ICP strategic aims. In particular this team will work closely alongside the Place Team and Primary Care Team to support planning and delivery.
The role supports the business in driving transformation as well as value for money in planning, commissioning and service.
The role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.
