POSITION SUMMARY:

The position directs the Admissions Manager and supports all Admission RNs, ensuring excellent customer service, and that all referrals are handled in a sensitive and timely manner - demonstrating professionalism, speed and accuracy. The position is also responsible for tracking business data daily, weekly & monthly and reporting lo management, obtaining all necessary documents needed for admission, developing strong and meaningful relationships with referral sources, and ensuring communication and status updates with referral sources in a timely manner. The Intake Coordinator also keeps detailed records on all referrals, admissions and conversions for accuracy and tracking ii against monthly goals; reporting back to the Admissions Manager on a weekly basis.

KNOWLEDGE AND SKILLS REQUIREMENTS:

  • Strong communication and customer service skills and the ability to establish and maintain effective Working relationships with staff, patients/families, referral sources, physicians, community partners, and other agencies/organizations
  • Excellent organizational skills and the ability to prioritize, problem solve and manage multiple tasks simultaneously
  • Ability to set priorities, research, problem solve and make independent decisions with minimal supervision
  • Ability to maintain highly sensitive and confidential material
  • Ability to initiate, develop and maintain relationships with referral sources and community partners including hospital discharge planners, physicians, vendors and nursing facility staff
  • Familiar with concepts and practices used in a hospice setting
  • Ability to maintain records and prepare related reports
  • Basic computer knowledge of MS applications such as Word, Excel, Outlook,
  • Working knowledge of departmental and organizational policies, procedures, and operations

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:

  • High school diploma
  • Minimum one year experience in healthcare or related field
  • Knowledge of hospital/physician referral process, end-of-life care preferred
