Description:

JOB REQUIREMENTS:

  • Education /Background: Minimum of a High School diploma or equivalent with at least one year of interviewing/case management experience. Bilingual preferred.
  • Job Knowledge: Knowledge of the legal/justice system preferred. Experience in case management/interviewing required. Must be able to build and foster relationships internally and externally. Must be sensitive to cultural differences within the agency and community. Fluent in Microsoft Office Applications (Word, Outlook Power Point and Excel).
  • Working Conditions/Physical Demands: Must be able to work in a fast-paced environment with interruptions. Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps. Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing. Requires driving and may lift up to 25 pounds.
  • Other requirements:
  • Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance.
  • You must be 21 years of age or older to drive on behalf of Catholic Charities.
  • Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.

Responsibilities:

JOB DUTIES:

  • Provides information about programs and departments to clients and community; explains program structure, class content and expectations.
  • Meets with prospective clients in timely manner and informs referral source of placement or non-placement, as appropriate. Coverage is offered during regular business hours to accept intake calls regarding program.
  • Completes intake paperwork for new clients.
  • Performs assessments with clients, interviewing and assessing needs; creates a case plan for period of service. Identifies community resources and assists in referrals.
  • Assists in the Development of individual program plans in accordance with procedure manual.
  • Maintains records, files, and statistics on all aspects of client participation in program, including development of individual files, daily class rosters and class evaluation paperwork.
  • Completes a monthly file review with program manager.
  • Monitors progress before, during and after program participation, completes an exit interview.
  • Upon completion of the program submits the file to administrative assistant who creates the paperwork for a motion to dismiss the case.
  • Assists in training and monitoring of volunteers and interns as assigned.
  • Arranges for class schedules and tracks attendance to the classes. Works closely with program staff to ensure classroom activities are running according to schedule and as planned.
  • Facilitates or co-facilitates educational groups.
  • Communicates regularly with contract representatives, accurately providing all documentation and statistics in a timely manner.
  • Manages the DIGNITY Diversion and Solicitation cell phone.
  • Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.
  • Attends and maintains training as required by agency.
