Responsibilities:

CLIENT SERVICE RESPONSIBILITIES

  • Respond to telephone/email inquiries from clients, referents, and other service providers on a daily basis acting as the primary contact for York Hills services.
  • Ensure that all referrals are processed through screening and waitlist criteria in a timely manner
  • Coordinate all aspects of the intake process including gathering consent and collateral information.
  • If not eligible for York Hills services, the Intake Worker makes recommendations to the client and/or referral sources regarding more appropriate services and assists them by following the warm transfer protocol.
  • Use agency-endorsed screening tools, including evidence-based and evidence-informed approaches in working with families.
  • Support wait list management activities.
  • Facilitate orientation to service workshops to clients prior to the start of service delivery.
  • Facilitate/support agency workshops as required.
  • Support families waiting for service and assign to Navigation as required.
  • Milieu Services Focus portfolio includes the above mentioned and:
  • Intake activities related to all day treatment inquiries and referrals.
  • Deliver Orientation to services in-person and remotely, group format or individual.

ADMINISTRATIVE RESPONSIBILITIES

  • Meet scheduled deadlines and program demands.
  • Ensure the client record is maintained as per organization standards.
  • Participate in quality assurance, evaluation and research activities as required.
  • Input all hours of work, requests for time off and expenses on a timely basis.
  • Maintain up-to-date registration and certification with the appropriate governing body.
  • Attend all team meetings.

ORGANIZATION RESPONSIBILITIES

  • Develop and maintain strong, positive relationships with collaborative partners, ensuring that mutually agreed upon expectations, such as the warm transfer protocol is met.
  • Participate in staff meetings, team meetings, conferences, training, and other meetings as required.
  • Build and maintain interactions with colleagues focused on individual, team and organization success.
  • Use individual supervision and peer supervision opportunities to develop professional goals, consult around cases, and share learning with others.
  • Follow agency protocols and policies around client safety.
