The Home First Intermediate Care Service supports patients to achieve their optimum independence by earlier discharge from hospital, and avoidance of hospital admission. You will assist in the delivery of high standards of health care and support patients within their own homes by following prescribed packages of care and therapy.
The vision of the service is to provide outstanding, patient focused, community-based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital; early assessment for care needs and avoidance of hospital admission.
Our operating hours are 7am to 10pm, 7 days a week.
Please note this post may be offered as a Band 2 Development post depending on qualifications and experience. You will be supported fully to develop into a Band 3.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Responsibilities:

With training, use skills from the professional areas of; Nursing, Allied Health Professionals and Medicines Management to undertake delegated tasks .
Apply competency-based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment/care plan.
Providing care to patients with deterioration in their health or function with a particular focus on admission avoidance to acute hospitals.
To deliver functional assessments, personal care, medication prompts, including mobility and appropriate exercise, personal and domestic ADLs, equipment provision
To carry out duties delegated and supervised by registered professionals
To provide a high standard of care

To manage own caseload of patients who require ongoing management and support with complex needs

Make judgements on patients’ response to prescribed treatment/care plans, and report findings to a qualified professional in line with competency framework..
Overcome barriers to understanding, e.g. patients’ with hearing or cognitive impairment.
Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.
Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
Be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately.
Work under guidance on specified tasks, taking direction from registered and non-registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner.
