Description:

This is an exciting opportunity to work in the Transfer of Care Hub as the Integrated Discharge Service Matron on a substantive basis. This role will provide day-to-day leadership of the IDS Operational Leads and the Integrated Discharge Service. There has been significant change within IDS and the service has grown considerably over the last year to meet the increasing demands. There are a large number of new staff within the service, as well as new Operational Leads, all of whom require support to meet their potential. This role will also have close oversight of the discharge delays in NBT, and the IDS plans and actions to resolve this. This role will be input into, and put into action, strategies and processes within the IDS on behalf of the Head of Integrated Discharge and Associate Director for Integrated Discharge.

There is huge potential within the service, and this role will be able to provide the direct senior support to ensure that this is realised, and support the Operational Leads to make sure the IDS teams are the best that they can be.

  • Line management and supervision for a team of 5 IDS Operational Leads
  • Escalation point for clinical and HR situations from the Operational Leads
  • Training, development, support for the Operational Leads
  • Assurance on behalf of IDS that discharge delays are appropriately and effectively managed across the Trust
  • Input into strategic decisions that influence IDS
  • Day-to-day operational alignment of the IDS teams and staff groups
  • Day-to-day operational implementation of strategic plans for the IDS
  • Complaint oversight and assurance on behalf of IDS
  • Ensuring that the IDS are closely linked with the other partners in the Transfer of Care Hub and benefits are being realised
  • Deputising for the Head of Integrated Discharge and the Associate Director for Integrated Discharge where required
  • Input into Discharge to Assess programme work that involves the IDS, and attendance where necessary at system working groups and meetings
  • Leading patient escalation meetings on behalf of the IDS and NBT

NBT Cares. It’s a very simple statement; one which epitomises how everybody across our organisation goes the extra mile to ensure our patients get the best possible care.
NBT Cares is also an acronym, standing for caring, ambitious, r e spectful and supportive – our organisational values.

And our NBT Cares values are underpinned by our positive behaviours framework – a framework that provides clear guidance on how colleagues can work with one another in a constructive and supportive way.
