Description:

The post holders will provide clinical support to our Bed Management Team supporting service users that have been placed in hospital outside of our own beds to return closer to home as soon as possible. The postholders will work alongside acute services and our community teams and agencies helping to overcome issues surrounding discharge from the acute wards within NSFT and placements in hospitals outside of NSFT.
This role will provide clinical management and co-ordinate complex mental health discharge pathways, whilst working collaboratively within a wider multi-organisational team alongside the Discharge Facilitators based on the acute wards. You will continue to develop and review processes as part of a co-produced service. In this role you will develop your experience and knowledge of discharge pathways and out of hospital provision as well as influencing discharge decisions.
The post holder will be expected to meet service delivery working Monday - Friday, 0900-1700, the role will require working across the acute sites and will require you to be able to travel to visit our service users who are placed in hospitals across the region attached to the post with a main base site identified.
Responsibilities throughout the patient pathway to discharge and also within the processes aligned to the Specialist Placement Panel.
The tasks below will be integral to the complex patient pathway and processes in place to support their safe and sustainable discharge. Both pathways and processes are subject to change as the whole service continues to develop. The post holder will be expected to keep informed of wider network pathway developments.
Work in conjunction with the Band 4 Discharge Facilitator staff on the wards to screen all admitted patients in conjunction with the ward staff using a screening tool in order to determine which patients present an indication of complex needs that will require further in-depth assessment to support appropriate discharge pathways.
Facilitate early discharge working alongside acute colleagues and professionals within aligned agencies to overcome barriers around discharge.
Supporting the acute wards and community teams to complete comprehensive, holistic assessments for patients with complex mental health needs who are suitable for rehabilitation and will require a funded placement with external providers upon discharge from an inpatient ward. Interpret and structure assessment outcomes into meaningful formulations of needs and problems that are understandable to the patient, their carer and the wider multi-disciplinary team enabling joint decisions about future care management and delivery options.
