Responsibilities:
MAIN DUTIES OF THE JOB
Responsibilities throughout the patient pathway to discharge and also within the processes aligned to the Specialist Placement Panel.
The tasks below will be integral to the complex patient pathway and processes in place to support their safe and sustainable discharge. Both pathways and processes are subject to change as the whole service continues to develop. The post holder will be expected to keep informed of wider network pathway developments.
Work in conjunction with the Band 4 Discharge Facilitator staff on the wards to screen all admitted patients in conjunction with the ward staff using a screening tool in order to determine which patients present an indication of complex needs that will require further in-depth assessment to support appropriate discharge pathways.
Facilitate early discharge working alongside acute colleagues and professionals within aligned agencies to overcome barriers around discharge.
Supporting the acute wards and community teams to complete comprehensive, holistic assessments for patients with complex mental health needs who are suitable for rehabilitation and will require a funded placement with external providers upon discharge from an inpatient ward. Interpret and structure assessment outcomes into meaningful formulations of needs and problems that are understandable to the patient, their carer and the wider multi-disciplinary team enabling joint decisions about future care management and delivery options.
