Description:
To take a lead role on mortality surveillance and monitoring, including analysing mortality data produced locally and nationally and presenting findings to Trust groups, including Summary Hospital-level Mortality Indicator and VLAD data. To work closely with the Medical Examiners to ensure compliance with the expected standard of Level 1 and Level 2 mortality reviews. To work closely with the Clinical Boards in order to ensure Mortality and Morbidity meetings have an agreed agenda and that the quality of the reviews meets the standard as laid out in Trust Policy and the Royal College of Surgeons good practice guide. To support the Clinical Boards to review, analyse and interpret their mortality data and understand the significance of themes and trends.
Support the Trusts patient safety and risk management function. To lead on the effective use of obtaining, interpreting and reporting on mortality data from data management systems. Using the risk management systems, support the Clinical Boards to use Datix for incident reporting and help them interpret this data in order to identify key patient safety and risk areas. To work closely with the Patient Safety Manager and Head of Patient Safety to ensure that appropriate risk management and patient safety related policies and processes are implemented effectively.
To deputise for the Patient Safety Manager as required. Provision of a wide range of education for clinical and non-clinical staff, including incident investigator training, Trust Induction and other patient safety related education. To take a lead role in the analysis/incident investigation process to ensure lessons are learnt and shared appropriately, particularly in relation to Trust identified Patient Safety Incidents (PSIs) and safety themes, aligned to the Patient Safety Incident Response Framework (PSIRF), of the National Patient Safety Strategy. Responsible for the supervision and management of other staff, plays a key management role in the department.
To contribute as a senior member of the Clinical Governance & Risk Department (CGARD) to broader developments as required.
