We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.
The Intensive Case Manager provides Intensive Case Management services to an individual caseload according to a housing first health functional, site-based supportive housing approach through site-based and mobile case management in the Hamilton Community.
This position is a unionized, full-time, permanent position. The rate of pay is $23.67 per hour. The start date is September 3, 2024.

Responsibilities:

  • Assists homeless individuals to locate, find and enter permanent leased units.
  • Documents and inputs information into client files, manually and in computer data bases to ensure accurate service statistics and client records.
  • Delivers services to program participants who are single homeless adults.
  • Develops implements and monitors case plans and sustainability plans.
  • Maintain regular, effective communication with social housing providers, administrators and private landlords, to ensure an integrated approach that will provide the best support possible for homeless individuals to maintain housing.
  • Facilitates case conference meetings, and participates in the co-ordination of appropriate supports required by participants to ensure an adequate wrap-around approach of services has been established.
  • Conduct regular visits with participants in their homes to provide supports specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration and community building life skills, grocery shopping, cleaning, etc.
  • Provides supports to participants in regards to their rights under the Residential Tenancy Act.
  • Completes pre and post-move inspections of housing units, and participates in negotiations with property management.
  • Other duties as assigned.
