Keep accurate and succinct records in the clinical system and on the patient tracker list. Attend daily discharge planning meetings. Facilitate patient flow related meetings and follow up actions.
Meet with patients and relatives to discuss discharge plans in-line with MDT recommendations. To ensure accurate and timely written records are kept which comply with Trust policy, reporting any issues as appropriate. Liaise with nursing, medical and pharmacy teams to ensure all medications are prescribed and dispensed prior to discharge. Escalate any any barriers that may result in a delayed discharge, via the agreed process To support all teams to ensure patient transport is organised and booked Demonstrate compassion in your wor
