Directorate overview
The Medicine and Emergency Directorate, (MED), which includes all specialties in Internal Medicine, was established in 2014 and has consolidated all specialties of medicine within a single clinical governance structure. It’s mission, aims and objectives focus on safety, (zero harm), quality, (excellent outcomes), patient experience, access and flow.
The Directorate has agreed a clinically-led management cycle for continuous improvement for adoption across its specialties. Clinical Specialty Leads have been appointed with a common structure and data-driven approach to specialty and directorate operations meetings.
Service overview
The cardiology department is a busy tertiary referral centre for a number of regional hospitals. The department consists of 9 full time consultants, 6 of whom are interventionalists. The department has a full range of adult cardiac diagnostic and therapeutic services. The non-invasive imaging includes exercise and dobutamine stress echo, trans-oesophageal echo, CT coronary angiography, SPECT CT and cardiac MRI.
The department provides a comprehensive coronary and structural service performing over 5000 procedures per year including 1600 PCI (500 STEMI, 24/7 service) and 200 TAVI. All major coronary modalities are utilised including FFR, intravascular imaging (IVUS and OCT), rotational atherectomy, lithotripsy and excimer laser. There is a weekly CTO session with both dissection re-entry and retrograde techniques.
There is a well-established transcatheter structural heart disease program with a highly successful TAVI programme, which includes transfemoral, transaxillary/subclavian and transcarotid. We currently perform mitral and tricuspid transcatheter edge to edge repair (TEER) repair and have an active adult congenital intervention programme with atrial septal defect and patent foramen ovale closure and left atrial appendage occlusion using advanced imaging modalities including 3D transoesophgeal echo (3D-TOE) and intracardiac echo (ICE).
St James’s cardiology department additionally run a yearly educational live course demonstrating the most up to date interventional cardiology skills, techniques and technologies to other Irish cardiologists.
Principal duties and responsibilities
Professional/Clinical
Be responsible for the safe and efficient assessment of patients
Work under consultant supervision to lead the resuscitation of all critically ill and injured patients
Liaise effectively with other specialty teams in relation to patient referrals
Supervise handover at the end of each shift.
Balance service demands with their personal educational needs and those of the doctors and medical students they supervise
Be supportive of junior staff, ANPs, nurses and other team members
Participate in clinical risk management, audit and research activities
Comply with departmental clinical guidelines
Support by the Consultants and their Registrar colleagues.
Comply with departmental leave policies and procedures
Demonstrate appropriate levels of professionalism in all aspects of your work and behaviour
Education and Training
Participate in mandatory and recommended training programmes in accordance with organisational / professional requirements
Maintain and develop professional expertise and knowledge by actively engaging in continuing professional education and development
Engage in planning and performance reviews as required with the Supervising Consultant

Comply with the policies, procedures and safe professional practice of the Irish Healthcare System by adhering to relevant legislation, regulations and standards

Administrative

Engage in service audit and demonstrate the achievement of the service objectives
Represent the department / profession / team at meetings and conferences as appropriate
Keep up to date with change and developments within the Irish Health Service

Eligibility criteria qualifications and/ or experience
Applicants are expected to have completed Basic Speciality Training in Emergency Medicine, Internal Medicine, General Surgery or Intensive Care Medicine.
Possession of a post-graduate qualification (eg MCEM, MRCS, MRCP), will be advantageous to the candidate, but is not a requirement for the post.
Candidates are expected to be ATLS or ACLS providers. Core requirements for this post include: a demonstrable commitment to medicine, good decision making, a broad clinical knowledge base and appropriate clinical skills, awareness of patient safety and risk management issues and the ability to work efficiently under pressure and as part of a team.
