Band 6 Learning Disability Intensive Support Team - Community Intensive Support Nurse / Practitioner (RNLD/RNM/OT)
Due to the expansion and development of the Learning Disability Service an exciting opportunity to work in the Intensive Support Team has arisen.
We are looking to recruit a highly motivated individual who has the relevant qualification and experiences or can demonstrate equivalent competencies within the following areas Occupational Therapy, Registered Learning Disability Nurse, Registered Mental Health Nurse, Behavioural Practitioner to join our growing team to meet the diverse needs of this cohort of individuals.
The Intensive Support Team (IST) is a community based service delivering specialist multi-disciplinary healthcare assessment and treatment services locally for individuals (fourteen years and over) who have a learning disability with / or presenting with a mental illness and / or severe challenging behaviour. The service provided is person centred and tailored to the individual and their environment, taking into account their ethnic and cultural backgrounds. We provide this through direct intense support, joint working with other services (e.g. Community Team for Adults with Learning Disabilities; Community Mental Health Teams; Community Team for Children) and / or through offering consultancy to other professionals involved.
The post holder will work as part of the Learning Disability Intensive Support Team within a multi-disciplinary framework, providing intensive assessment, formulation and intervention for clients who present with behaviour that challenges and / or mental health issues.
The post holder will provide clinical leadership and evidence-based working in partnership within the IST, wider Learning Disability Service and external services to ensure a seamless pathway for clients. Within this comprehensive whole team approach, we aim to work towards personal recovery by using creative and innovative solutions to address conflict and differences whilst providing interventions that will enable individuals to gain control over their lives and reduce levels of vulnerability, challenging behaviour and effects of mental illness.
Post holders will have experience of working with people who have a learning disability and either mental ill-health and/or complex behavioural needs.
Successful candidates will need to have a genuine interest in working within a multi-disciplinary team providing Assessment and Intervention for the defined group of individuals. You will be expected to work within a values driven, recovery based approach that imbeds the Positive Behaviour Support framework.
The Intensive Support Team work in close partnership with the client and family / carers, and other services, such as Social Services, generic Health Services, independent and voluntary services, Advocacy Services and Commissioners. Agreed commitment from all involved will be obligatory in order for identified outcomes to be realised.
The Intensive Support Team operates 7 days a week, Monday to Friday 08.00 to 20.00 hours, weekends and Bank Holidays 9-5, with flexible working outside of these hours where required, supporting individuals to remain in their home and community environment wherever possible, providing appropriate advice and support, assessment and intervention, behaviour management, training to families and carers, in line with the Transforming Care Agenda. The team structure and hours have been developed in line with the national model and in order to meet local needs.
The post holder will work an average of 1 weekend in 5.
The Intensive Support Team is an integral component of the Northamptonshire Learning Disabilities Service, including CTPLD’s, Social Care, CHC and CCG. Referrals to the team occurs when current work being undertaken in the supporting services is insufficient to meet the individuals needs and or where behaviour that challenges persist or becomes more serious to the point where a person and/or others are at risk of serious harm and where individuals are at risk of exclusion from ordinary community settings (including admission to hospital). IST will then work alongside teams involved to provide intensive support for a defined period.
The Transforming Care agenda is a National driver for the Learning Disability Service; the post holder will ensure the role of the IST provision is integrated and cohesive within the countywide community Learning Disability Service, to provide a seamless pathway for individuals.
