Description:

Dimensions and Context of Role The Keyworker Co-coordinator will work alongside the Clinical Team Managers to ensure the effective implementation of the Key Worker Program, an NHSE/I funded pilot to improve outcomes for CYP with Learning Disabilities and/or Autism. The post holder will lead on the implementation and evaluation of the project and will be responsible for the administrative tasks for the clinical staff through effective organisation and co-ordination. The post holder will collate and report the clinical data and KPIs required, offer project lead/business support to the Clinical Team Managers through coordinating action plans, supporting the implementation of governance processes, collating performance data and supporting the team in the development and operational effectiveness of the service. They will support the clinical team to assist in tasks which impact on the effective running of the service that might prevent them from undertaking their clinical duties.
These tasks will include maintaining the Dynamic Support Register, coordinating clinical meetings and appointments. The coordinator will manage new referrals, assisting in the process to determine whether the CYP meets the criteria for the keyworker and co-ordinate the team to run efficiently. This will require excellent communication skills and the ability to form good working relationships between agencies. The Keyworker Co-Ordinator role will sit within the CAMHS Admin team and therefore have line management and supervisory duty for other administrative staff within the service.
Key Tasks and Responsibilities of the Post To assist the Clinical Team Managers in the management of the Keyworker program by offering project lead and business support. This will involve undertaking tasks to operationalise the team. To support governance, performance and reporting processes. To collate the relevant information required in a format that can be presented to stakeholders.
Managing communication on behalf of the team, taking referral information, coordinating the teams clinical activity, arranging multi-agency meetings, co-ordinate and facilitate the writing of clinical reports/letters and onward referrals. Assisting with the management of the Dynamic Support Register (DSR) with the designated lead. This will involve keeping it up to date, reporting the relevant information for monitoring purposes and ensure that there is oversight of all the CYP open to the team. To provide line management and supervision to administrative staff within the CAMHS Admin team, ensuring staff are supported and are up to date with mandatory training, induction, including probationary periods and annual appraisals.
In addition, the coordinator will undertake the administrative tasks for the team. This will include managing incoming calls, typing where required and will support the clinical team completing tasks that free up time to care. The coordinator will be expected to demonstrate excellent and supportive customer care during any call, particularly when assisting with the needs of families who contact the team and other multi-agency professionals. The worker will ensure that quality and performance monitoring data collated and available monthly.
The coordinator will be expected to attend meetings relating to the keyworker service, book rooms, prepare agendas and complete minutes as requested. The coordinator will form a good working relationship with our co production colleagues in Devon, linking in when required. The coordinator will assist in service changes, training and development of others. To know how to report accidents and untoward incidents and to do so promptly.
To obtain knowledge of Trust policies, procedures and practices and carry out duties in accordance with them. To be aware of legal issues and have an understanding of the Mental Health Act, Mental Capacity Act and Transforming Care. The coordinator will ensure that quality monitoring for the team is delegated and carried out as required. The coordinator will be able to access staffing information for the team and be able to maintain e-roster.
To be willing to participate actively in appropriate in-service training programs which the managers believes to be of benefit to the individual. To assume co-responsibility for maintaining skills and knowledge levels. Undertake specific responsibilities designated by the Clinical Team Managers. To undertake mandatory and other training as directed by the Trust.
