An excellent opportunity has arisen to recruit a Community Psychiatric Nurse/Care Co-ordinator into our busy Community Mental Health Team for Later Life
The area covered by the post is North Later Life CMHT Manchester
We are looking for an experienced, committed and motivated professional to work with older people who are living with dementia or functional illnesses.
We are a friendly and supportive team and encourage professional and personal development
Care Co-ordination under CPA
Complete Memory Assessments (training will be given)
Administering depots
Complete initial assessments
Participate in Duty Rota
