DETAILS
- Department: Grants, Pine Heights Location
- Schedule: Monday-Friday. No on-call. No weekends. No holidays
- Hospital: Ascension St. Agnes
- Location: Baltimore, MD
- Salary: $31.28-$46.92
Actual compensation offered will vary based on factors including applicable licenses, certifications, education, experience, location, qualifications and comparison with associates in similar roles.
REQUIREMENTS
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate’sdegree/Bachelor’s degree with 1 year of experience OR 5 years of applicable cumulative job specific experience required. 2 years of leadership or management experience preferred.
QUALIFICATIONS:
- Additional education in health or related fields is a plus.
- Strong familiarity with navigation software systems.
- Proven ability to train and coordinate team efforts.
- Resilient, solution-oriented mindset with the ability to handle challenges effectively.
- Excellent organizational skills and attention to detail.
Responsibilities:
RESPONSIBILITIES
Plan, organize and lead the operation and activities of community health programs. Monitor
program compliance toward goals, financial performance, outreach activities and collaborative
partnerships.
- Lead day to day operation of sites including staffing, fiscal performance, compliance with goals and clinical standards. Provide updates and input for work team and individual performance evaluation.
- Create, compile, and analyze statistical and/or demographic data reports/graphs and distribute as appropriate.
- Provide oversight to yearly outcomes and department strategic plan, offering guidance and direction to the work team. Serve as coach and resource to other team members.
- Collaborate with leadership on process improvement initiatives and quality initiatives.
KEY RESPONSIBILITIES:
- Train and mentor team members, fostering a collaborative work environment.
- Organize and coordinate the needs of associates to optimize program efficiency.
- Utilize navigation software systems to ensure timely and effective transportation solutions.
- Manage multiple facets of program operations, demonstrating strong organizational skills.
- Engage with community members, providing patient-facing support and solutions.
