Description:

This community-based role is to lead a frailty interface service between primary and secondary care. The Urgent Community Response (UCR) team are an integrated multidisciplinary team, providing a rapid response and frailty assessment for patients in the Lewisham Borough, as an alternative to hospital admission.
The lead role will manage a team of Frailty Specialist and Advanced Clinical Practitioners working within the team. They will need to demonstrate leadership, team working and autonomy in holistic frailty assessment and management. They will also be expected to continue to advance and develop this progressive community service, supported by a Consultant Geriatrician and Head of nursing.
The postholder will require a proven track record of strong leadership skills, as well as be trained in advanced clinical practice to support and provide direct patient care. The role involves professional development of the team, through education and training, as well as service development, including research and audit.
UCR was established in April 2021 and there is potential for development and expansion of this service, including supporting complex patient stepdown from hospital, expanding referral pathways, and widening the scope of the current Outpatient Antibiotic Therapy (OPAT) service to enable intravenous treatment in the community.

To act as a role model of expert clinical practice by participating in the provision of care to a definable patient group.
To act as an expert clinical practitioner in the clinical setting, initiating advanced practice skills, clinical decision making. and appropriately investigate, diagnose and treat.
To independently prescribe medications, treatments, order investigations and manage referrals onto other specialities where appropriate.
Spend approximately 20% of their time in the clinical setting, working with a patient caseload – independently assessing, teaching, implementing, evaluating and discharging patients.
To work autonomously and accountably; demonstrating expertise, which improves the quality of care, health outcomes and experience of patients
To take clinical initiative in response to the changes and developments in patient need.
Ensure that appropriate information is provided to patients and relatives.
To attend and participate in regular MDT meetings.
To work in partnership with other care providers to broaden the provision of care offered.
To maintain accurate, clear and concise written records on the electronic patient management systems, adhering to patient confidentiality.
To develop evidence based clinical standards and provide a framework for changing practice.
