Description:
Responsibilities:
ROLES AND RESPONSIBILITIES:
- To be responsible for the clinical pharmacy service and take a lead role in medicines optimisation within ADHD and mental health for the company.
- Provide leadership to mental health formulary development and management
- Conduct evidence-based searches, evaluate the evidence and present to the Medical Director/Lead Clinicians for consideration for entry into the formulary
- Provide expert advice on the most current and relevant legislation and guidance related to private prescribing of controlled drugs and mental health medication.
- To support the work of the Drug & Therapeutics Committee including development and implementation of new policies and guidelines, managing the introduction of new drugs and safe medication practice
- Provide support, clinical expertise and educational sessions
- Support the transition of patients across care settings so they receive the required level of care to support them in taking their medication safely in the most appropriate care setting.
- Provide expert advice around prescribing in Mental Health conditions where appropriate and required to do so
- Utilise local intelligence and data to target areas of Mental Health Prescribing to audit and review. Produce action plans and drive implementation changes as indicated by results.
- Support the care of patients, with regard to medicines, when they transition between services.
- Carry out medication reviews in patients / patient groups identified as being a priority.
- To feedback the outcome of the auditing to individual clinicians (where appropriate) and to the Managed Clinical Network
This would be 8 hours of commitment and offered on a part time basis with hours that can be worked which suit you.
