Description:

Our Coventry Community Neuro and Stroke Rehab Services (CINSS) are looking for a highly motivated Physiotherapist to join their interdisciplinary team.
The service is a community based interdisciplinary therapy team, who support the timely discharge of patients from hospital. Our patients have ongoing rehabilitation or neuro management needs following a new or worsening presentation of a neurological condition, both stroke, and non-stroke.
The team work collaboratively with acute services, rehab and community networks and third sector services to deliver a patient-centred; goal focused approach to rehabilitation. Our team is an integral part of the stroke pathway, in reaching into both stroke and in-patient rehab settings.
This post will offer you the opportunity to consolidate and develop neurological and wider clinical skills. Your skills and experience will be integral to our quality improvement and service developments.
As a service we are committed to supporting continuous development of clinical skills through joint working, access to a comprehensive in-house training program, and opportunities to attend both, internal and external courses.
We will accept full and part time applicants (minimum 30 hours).
An inclusive team approach and excellent communication skills are essential. You will be expected to work independently and effectively within the Community Stroke and Integrated Neuro Rehab Service to ensure safe and effective management of these patients in the community. You will be able to prioritise your workload and time effectively to support an integrated approach.
You will be required to be part of the Interdisciplinary team, supporting patients to meet their goals in a community setting.

1. To be an integral part in the provision of an effective and high-quality community stroke and neuro rehab service, liaising closely with therapists and nurses from University Hospital (UHCW), community therapy pathway and community discipline specific services.
2. To follow defined stroke and neurological care pathways and other relevant rehabilitation pathways as required.
3. To utilise a comprehensive range of evidenced based specialist assessments and treatment to patients, participate in triage, to determine whether the referral is appropriate and establish any clinical risk.
4. Through highly specialist clinical reasoning and practical application determine specific therapeutic needs, taking into account holistic and interdisciplinary requirements.
5. To effectively manage and prioritise a large caseload of patients with complex conditions based on the nature, complexity and urgency of each individual case.
6. To have an advanced knowledge of neurological conditions.
7. To support the Clinical Lead with the maintenance of professional leadership for the Occupational Therapists within the team.
8. To support the Service Lead and Clinical Lead in the day to day management, quality and activity monitoring systems and service development.
9. To support the Service Lead and Clinical Lead with development of the service.
