Description:

Play a key role in the delivery and coordination of care, within the course of their duties the post holder will work independently within established policies and procedures and provide care as delegated by a registered healthcare practitioner. Contribute to the on-going assessment and coordination of care of the service user providing information to inform assessments and discussing any potential changes to planned care with Registered Practitioners. This includes being responsible for some elements of patient assessment, undertaken in conjunction with the registrant, implementing programmes of care, and modifying programmes of care as delegated by a registered practitioner. The service navigator will be allocated a group of patients/patient to care for, in the same way that patients are allocated to a registered practitioner.
The supervising registrant will undertake elements of care that sits outside of the band 4 scope of practice. Elements of activity and responsibility under the supervision of a Registered Practitioner will include: Reviewing patients independently and with other practitioners as part of planned and unplanned visits Mental Capacity Act and Best Interest activity Identifying and co-ordinating activities/agency/respite Transition planning Visiting prospective placements as appropriate Attending ward rounds/ Multi disciplinary team meetings as appropriate Participate in evidence collation for scrutiny panel Lead on collating information for brokerage applications/alternative care provision & benefits review/ application Liaising with stakeholders for Continuing Healthcare (CHC) assessment and Decision Support Tool completion Participate in Care and Treatment Plans/117 reviews and the Care Programme Approach Coordinating, minute taking and writing up MDT meetings Liaising with and between family, commissioners and other professionals Coordinating care to meet individual need and responding to requests as required and appropriate Undertake referrals to other members of the MDT as well as to other agencies as appropriate Liaise with other services/agencies as appropriate. In conjunction with a Registered professional supervisor organise own caseload/workload including all administrative tasks and data entry. To follow Health Board and Departmental Policies and Procedures in managing compliments, complaints and incidents.
