JOB OPPORTUNITY

Compass Rose Care is seeking a compassionate and highly skilled Licensed Practical Nurse (LPN) to join our team as a Care Provider. This position provides hands-on care provision for our discerning clientele. The ideal candidate will be a strong clinical LPN, an empathetic caregiver, and someone committed to providing exceptional home care services.

REQUIRED QUALIFICATIONS:

  • Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).
  • Minimum of 3 years of clinical nursing experience, including at least 1 year in home care or community health settings in Canada, preference will be given to those with experience in Calgary, Alberta.
  • Expertise in caring for clients with complex medical requirements (i.e. CVADs, wound care, IV therapy, home dialysis, etc.) and client teaching.
  • Ability to communicate effectively, both written and verbally, in the English language, nurturing excellent human relations with clients, families and other team members.
  • Strong clinical skills and decision-making abilities in complex situations.
  • Ability to work with minimal supervision and to evaluate and meet client care needs, demonstrating initiative and problem-solving skills.
  • Dependable, responsible and an ability to work well with others in a team environment and independently. Demonstrates a positive attitude necessary to meet the goals and standards of the company.
  • Proficiency with EMR systems and other technology tools including Microsoft 365 suite of products.
  • Current BCLS-HCP certification.
  • Valid driver’s license and reliable transportation for daily client visits.
  • Physically capable of lifting up to 50 lbs and performing caregiving tasks

Responsibilities:

· Participates in and adheres to continuous quality improvement standards.
· Participates in assessing the needs of individual clients and follows care plan to promote maximum client independence.
· Participates in ongoing assessments on the health and wellness needs of individual clients and makes recommendations, as necessary, regarding the clients health status and and their needs. Stays up-to-date on services provided and works with clients, their families and the care team to meet the clients’ needs.
· Ensures the client’s file is established, with proper documentation and safe practices related to medication administration processes.
· Carries out planned assignments as designated by the Clinical Educator and Care Coordinator and/or Clinical Supervisor or designate, for effective client care, taking into account the physical, spiritual, social and emotional needs of the individual.
· Distributes medications and provides treatments to clients in accordance with current and approved standards of care, policies and procedures.
· Provides direct client care, in accordance with current and approved standards of care, policies and procedures.
· Complies with infection control protocols as directed, e.g. outbreak management.
· Maintains accurate records of nursing observations and procedures performed. Completes all required reports.
· Assists in maintaining a neat, clean and safe environment for clients and their families. Recognizes safety hazards and takes appropriate action.
· Assists with safekeeping of all medication and nursing solutions in the home.
· Reports all incidents and changes in client status to the Clinical Educator and Care Coordinator and/or Clinical Supervisor
· Participates as a member of the multi-disciplinary team in the development of, and implementation of, a care plan for clients
· Assists in monitoring and evaluating outcome of specific interventions regularly and offers information or suggestions for improvement.
· Utilizes equipment and supplies effectively and in accordance with policies and procedures, manufacturer’s instructions, and health and safety regulations.
· Assists with other duties to ensure the smooth functioning of the client’s home, including housekeeping and laundry duties, maintains adequate stock of supplies, as required.
· Maintains effective communication, both written and verbal with co-workers. Uses the appropriate lines of communication with supervisory staff, and good interpersonal relations with clients, families, physicians and other care providers.
· Performs other duties as assigned by the Clinical Educator and Care Coordinator and Clinical Supervisor
· Complies with all corporate policies and procedures.
· Completes all required mandatory training/education.
· Complies with the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Material Information System/Globally Harmonized System. This includes recognizing health and safety hazards, reporting incidents, fulfilling responsibilities under the applicable legislation, as well as participating in in-services.
