The Survive and Thrive Program is a community-based, individual, and family support service that aims to build resilience in families experiencing significant vulnerability. The program provides early intervention, prevention services, and resources that will improve children’s development and well-being while supporting the parenting capacity of participants.
Responsibilities:
We seek a dedicated and compassionate Lifetime Learning Aboriginal Family Case worker to join our dynamic team. This role empowers Aboriginal families with children aged 0-5 through group and individual support.
You will be instrumental in delivering comprehensive services to improve social and economic inclusion and strengthen community ties. Your responsibilities will also include:
- Provide integrated support services, emphasising prevention and family empowerment.
- Develop and deliver culturally relevant, evidence-based parenting services.
- Foster relationships with Aboriginal service providers, organisations, and community leaders.
- Develop tailored case plans aimed at achieving family goals.
- Advocate for families within the community, enhancing parenting experiences and program objectives.
- Maintain confidentiality and ensure best practices in service delivery.
- Collaborate with external organisations and participate in community-driven activities.
- Commit to the principles of the Sanctuary Model, Anglicare’s Values, and Code of Conduct.
