Description:
Work in a way that promotes dignity, respect independence. Contribute to developing a culture of parity whereby all protected groups are afforded fair and equal treatment. Work with individuals that use the service, experts by experience, Peer Support Workers and Carers to help ensure optimum delivery of care. Work closely with the Discharge Team and wider MDT to ensure individuals and Carers are involved in care and discharge planning.
Identify Carers at the point of admission. Empower individuals and Carers to make decisions. Be a point of contact for staff and Carers within the mental health inpatient settings. Meet with Carers within the home environment, community settings and mental health inpatient settings.
Communicate sensitive information which is often complex in nature. Work closely with a number of agencies and disciplines and proactively promote relationships in order to meet the needs of individuals and Carers. Review, develop, implement and deliver Lived Experience and Carer outcomes. Complete referrals for Carers Assessments.
Contribute to Multi-disciplinary Needs Assessments. Undertake surveys for Carers and individuals that use our service. Act as a catalyst for change through feedback from Carers and individuals that use our service. Ensure feedback measures, such as surveys, complaints and compliments are linked with service user participation.
Develop and update all materials related to Lived Experience and Triangle of Care. Provide verbal and written information and resources. Ensure current literature and initiatives regarding Lived Experience and Carers are distributed and advertised across the mental health in-patient services and are implemented accordingly. Maintain up to date knowledge of evidence-based practice relevant to the role.
Signpost to services within the voluntary and third sectors. Assist to pilot new ideas and innovations in relation to Lived Experience and Carers. Take part in coproduction activities to develop services. Ensure changes are monitored and evaluated.
Be able to evaluate their own work and to identify gaps in the service to highlight where improvements can be made. Oversee and participate in audits and contribute to service improvement initiatives. Co-facilitate support groups. Provide practical and emotional support through support groups, forums and one to one contact.
Become familiar with the social background and risk history of individuals that use the service before contact is made with the Carer. Promote and support the Carers Leads within the Livewell Southwest mental health inpatient services. Develop, attend and contribute to forums for Lived Experience and Triangle of Care. Provide advice, support and mentoring for peer support workers.
Gather data and maintain databases. Undertake specific responsibilities designated by the Discharge Team Leader and senior managers. Maintain confidentiality in respect of the person. Report any safeguarding concerns.
Know how to report accidents and untoward incidents and to do so promptly. Have knowledge of organisational policies, procedures and practices and carry out duties in accordance with them. Be aware of legal issues and have an understanding of the Mental Health Act. Be willing to undertake mandatory and other training as directed by the organisation and Line Manager.
Assume co-responsibility for maintaining skills and knowledge levels. Participate in personal development using the appraisal system. Receive line management support and supervision. Effectively record information on systmOne.
Behave in a manner that does not bring the organisation into disrepute. 5.1 Responsibility for People Management Undertake the immediate line management for a designated number of staff. Support Practice Supervision and training for Peer Support Workers. 5.2 Responsibility for administration To gather data and maintain databases.
