Responsibilities:
- Work alongside people with disabilities to increase their independence and participation.
- Build trusting relationships with participants, families, and carers.
- Support participants in navigating community resources and the NDIS system.
- Help develop community connections, support plans and access.
- Collect and record accurate information within the client record management systems.
- Maintain compliance with NDIA performance measures and timeframes.
- Work in collaborative working environments to support people in the community at information hub
