Description:

This post is for 0.4 WTE Locum Consultant Psychiatrist working within the North West Camden Core Community Mental Health Team as work progresses with the transformation of community mental health services in line with the NHS Long Term plan.
It is available from October 2024 for a period of one year fixed-term. This is an existing post within this service that will become vacant when the current post holder moves to a different service within the trust.
The post-holder will be part of a multidisciplinary team which includes both statutory and voluntary sector roles and provides holistic care to patients in a primary care setting aligned to the South and Central Primary Care Networks in Camden. The post holder will work alongside two other consultant colleagues (full-time and 0.6WTE) and other members of the multi-disciplinary team to carry out assessments.
The post holder will work with GPs within the South and Central Camden Primary Care Network. The total GP population covered will be approx.: 100,000.
The clinical work takes place within primary care, alongside GPs and at the Core Team base.
Referrals will come from the 14 GP practices in the North West Camden PCNs. Following initial triage, signposting and advice giving to referrers, approximately 60-70% are offered appointments with the team. The post holder will work alongside two other consultant colleagues and other members of the multi-disciplinary team to carry out assessments.
It is anticipated that the post holder will conduct 1-2 assessments per week and hold a short-term caseload of up to 30 patients at any one time with GPs remaining as main case holders within this collaborative model.
Team managers maintain an overview of team and individual practitioners’ caseloads.
It is expected that the post holder will participate in active management of their own caseloads in conjunction with team managers.
The team base is currently located at JML Building, Regis Road, London, NW5 3EG, but it is planned that this will relocate to a primary care or community base in Camden. The primary record-keeping system for the service is RiO.

  • To provide advice and support to South Camden GPs, and other practice staff, regarding the assessment and management of the mental health difficulties of patients registered with their practices. This may be done through case discussions, joint consultations and bespoke training as appropriate.
  • To provide consultant input and clinical leadership to the South Camden Core Team. To assist with diagnosis, formulation, medication reviews, management plans, risk assessments, crisis plans and self-help plans as required for these patients referred to the team.
  • As necessary to provide direct clinical assessments of patients registered with the South Camden GP practices, and to provide follow up as appropriate.
  • To assist in the prioritization (“triage”) of new referrals to the service.
  • To maintain effective communication with GPs, primary care staff, patients and their carers.
  • To work flexibly within timetabled working hours, so that urgent clinical matters can be prioritised. To carry out Mental Health Act and domiciliary assessments as required.
  • To play a key role in implementing improved linkage between primary and secondary care services.
  • To liaise and work closely with local IAPT services (also provided by C&I) and other primary care providers in order to provide a joined up primary care mental health service.
  • To liaise and work closely with local voluntary services as part of the new model of core community teams.
  • To provide clinical input, where necessary, to multi-disciplinary case conferencing around difficult patients being managed solely in primary care.
  • To engage with local service user participation models.
  • To participate in management activities and governance processes related to the Practice Based Mental Health service, including audits and quality improvement projects.
  • To play an integral role in the performance of the team, the further development of the service, and the development of clinical pathways and priorities of the service line.
