An exciting opportunity has arisen at London North West Healthcare Trust for an enthusiastic, motivated and dynamic individual to join the Specialist Palliative Care team as a Macmillan End of Life Assistant Practitioner. The Trust’s palliative care service is responsible for delivering high quality end of life care to patients and carers based in the acute hospital setting, so if you thrive on a challenge and feel you can make a real difference to the care of our patients, this post may be for you.
The palliative care nursing team are looking for a Band 4 Assistant Practitioner to assist them in providing high quality care to patients entering the end of their lives. The role will include a mixture of patient and carer assessment, generic staff training around end of life care and liaison with treating teams around symptom management. Candidates should be strong patient advocates with good communication and clinical skills. The role involves elements of discharge planning and data management.
The Macmillan End of Life Assistant Practitioner will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with a life limiting illness (both malignant and non-malignant disease) and their carer’s.

The overall purpose of this post is to support the delivery of the End of Life Care Strategy within LNWH. The post holder will be involved in the delivery of nursing care for patients in the last phase of life with the level of knowledge, skill and competence beyond that of a traditional healthcare assistant. You will support and contribute to education and training of registered nurses, healthcare assistants and other members of the multi-disciplinary team (MDT), with regards to the specific needs of patients and their families/carers at the last phase of life.
Working under indirect supervision, as part of the nursing team, you will have responsibility for delivering nursing care to patients within a delegated caseload.
You will participate in multidisciplinary working and the development, implementation and evaluation of the delivery of health care services.
