Provide leadership and support for the Haematology Cancer CNS Teams cross-site Work autonomously and demonstrate advanced practice by managing a caseload of patients and demonstrate advanced practice in the holistic assessment Lead the development of Personalised Care for haematology cancer patients which will include the further development of the Supported Self Management programmeand play an active role in the delivery and design of Moving On and Health & Wellbeing Events Promote collaborative working relationships and effective communication between all members of the MDT to improve the management and outcomes of patients within the specialist area Facilitate effective communication and co-ordinate referral of patients and carers to appropriate support services and community Trusts to promote a seamless service and thus reduce/avoid admissions and length of stay to the acute trus
