Description:

The post holder will be expected to work autonomously and as part of a team, maintaining patient centered care and acting as an advocate, liaising with MDT across the South Wales Trauma Network to provide gold standard holistic care for patients following traumatic injury.
The post holder will be expected to work autonomously utilising own clinical knowledge and skills and as part of a team, maintaining patient centered care and acting as an advocate, liaising with MDT and coordinating care.
This role involves co-ordinating the care for major trauma patients across specialties to include Surgery, Accident and Emergency and Trauma and Orthopaedics
The day to day activities will include attending daily trauma meetings, identifying major trauma patients and carrying out patient assessment, investigations, management - including therapeutic and non-therapeutic. Co-ordinating patient follow up and carrying out telephone consultations to outpatients when required.
The successful candidate will need to show high level communication skills to other health care professionals, patients and relatives
Opportunities will also be available to carry out research in relation to major trauma services in order to shape the future of the service in CTMUHB
Secondment for 2 years to meet the needs of the service

The post holder will be expected to provide and receive highly complex information.
Discussing major trauma care pathways and plans with colleagues and patients.
Access written and computerised information.
Establish effective communication networks and strategies within the UHB, with the Major Trauma Network and nationally.
Establish effective and seamless communication throughout the patient’s journey including emergency department, radiology, wards, theatres and critical care.
Ensure that the Major Trauma Director and Deputy Major Trauma Director are kept informed of major trauma activity and potential problems that may have been identified.
Participate actively in trauma-related meetings.
Take every opportunity to expand own practice in line with principles and guidelines published by the Nursing Midwifery Council.
Work within the clinical governance framework and to take responsibility as part of the role.
Work with Major Trauma Director to develop and maintain an up to date risk register.
Prioritise own time to be available to coach and mentor others.
The post holder will be making judgements involving complex facts and situations which will require analysis, interpretation and comparison of a range of options:
Assessment and review of patient needs, Reporting and escalation on patient conditions, Make direct referrals to relevant specialty teams.
