We are seeking a committed team player to join our Malignant Haematology Multidisciplinary Team. This is a full-time post. You will be an enthusiastic, conscientious team player, with the ability to work under pressure. We are looking for someone with excellent written and communication skills and a proven ability to manage time and organisational skills. We encourage free thinking and will happily work with ideas from any member of the team. A caring and sensitive attitude is essential as you will be in frequent contact with patients and their families.
The role of the Pathway Navigator is integral to the smooth, coordinated and efficient running of the malignant haematology pathway. Ensuring that patients and their carers are supported and signposted through the complex pathway.
The post holder will improve patient experience by providing an accessible, friendly nonclinical contact for patients and their families. You will provide support and information throughout the entire journey, thus reducing anxiety and providing reassurance. You will provide information on interventions/tests, ensuring the patient arrives fully prepared.
It is essential that the post holder can exhibit a clear understanding of confidentiality. You will have significant NHS clerical and administrative experience. With the ability to manage the IT systems used within the service (PPM, Cerner). You will also be able to demonstrate an understanding of cancer targets and haematology patient pathways.

The purpose of this role is to support the Haematology team to implement the timed optimal pathway for patients with suspected haematological malignancies, to achieve definitive diagnosis. This role has been specially designed to support the malignant haematology team and will require the post holder to evolve to support the pathway as it continues to refine over time. They will provide comprehensive support to the Haematology multidisciplinary team through the co-ordination of primary and tertiary referrals and improve patient experience by providing support, information and reassurance to patients throughout the cancer pathway.
The post holder will be required to work across a range of IT systems to coordinate the booking of patient appointments in the timed optimal Malignant Haematology Pathway. They will work both directly and indirectly with patients and their families/carers to assist in coordinating the care of patients through their hospital care journey, providing the patient with the relevant information and ensuring patients arrive fully prepared for any interventions . The post holder will provide psychological, emotional and practical support, acting as a point of contact for patients and their families and as a liaison point between referral units.
The post will require the coordination of appropriate services and will facilitate the pre-booking of diagnostic test bundles to maximise the delivery of a streamlined diagnostic and treatment pathway, thereby improving quality of care and experience for patients and their carers and diagnosing cancer earlier. In conjunction with this the post holder will direct patients and their carers already on the cancer pathway to the correct services required.
The post holder will be responsible for identifying the causes of any delays in the patient journey that may affect the patients’ prognosis and/or the delivery of cancer waiting time targets, escalating problems to the Clinical Business Unit management team in a timely manner so that remedial action can be taken.
It is expected that the post holder can work autonomously, using their own initiative and within a specialised team. The post holder is expected to be experienced within the hospital setting, and educated to a level required to perform the role or equivalent experience. The post holder will actively support and contribute to the requirements of the Malignant Haematology service and ensure compliance with performance targets and quality standards required at a local and national level
