The Manager, Clinical Programs on the Transplant team will drive the development and execution of MedSleuth’s care offering to transplant patients. This is an exciting new opportunity to help build an offering that addresses a clinical market need to improve clinical outcomes. This role will work to leverage DaVita’s existing clinic footprint, IT systems, and care management expertise to stitch together a comprehensive care management service to patients.
This is an incredible opportunity to build services that will directly improves patient experience and engagement. This Manager will get to build an entirely new capability that will create meaningful clinical impact for patients.
Skills required:
- A continuous improvement mindset, adopting an iterative approach to testing
- Collaborative approach to closely coordinate with the clinical provider community, local Transplant programs and community Nephrologists
- Ability to Interpret and summarize outcome data to make daily, weekly, and long-term modifications to create efficiencies
- Outcome based mindset to ensure our processes are enabling us to achieve the right outcomes
- Applies innovative and strategic thinking that will lead to program improvement and drive connections across other Village programs
- Strong leadership and team management experience to inspire and motivate others
Role & responsibilities:
- Identifies strategic insights and connections for how the program can create value
- Drives operational, financial, team and clinical performance of your business KPIs
- Directly lead a team of clinical and non-clinical teammates who provide key services to patients
- Evaluates performance of current initiatives to identify new opportunities
- Leads the development, piloting, deployment and measurement of outcomes for processes and workflows in this new offering
- Conducts financial planning and analyses to assess path to viable economics
- Works closely with local operators to manage key Nephrology relationships as we deploy this offering to patients
- Conveys findings in a cohesive story, including summarizing insights to share ideas with leaders
- Builds consensus for changes to make in how we deliver post-transplant support to patients – considering impacts for stakeholders involved
- Builds mechanisms in place to monitor patient satisfaction for how they are experiencing the services provided to them
- Ensures the program is effectively meeting high quality clinical standards as NPs receive lab results timely and abnormal results are managed effectively with the right clinical stakeholders
