Join DaVita, affectionally called “The Village” as the PD (Peritoneal Dialysis) Retention Manager.
The Peritoneal Dialysis (PD) Retention Manager, is responsible for home patient retention through preventing controllable clinical loss within one division. This includes coordinating both at the facility, region, and divisional level to monitor potential loss/high risk patients, drive the development of patient-specific care plans, coordinate with facility IDT members to develop interventions for high-risk patients, and support modality retention training and education for Home teammates. While the PD Retention Manager will oversee the entire patient population in the division (~500 patients), it will be their responsibility to use data and reporting (i.e., PD AI output, electronic treatment records, Village reporting, etc.) and conversations with clinic teammates to identify those patients most at risk of modality loss and/or hospitalization and develop appropriate interventions. Success will be measured by overall improvement in home modality loss rates and other home clinical and process metrics (including but not limited to peritonitis rates, AI Loss, etc.)
This position is home based in Manassas, VA but will be expected to travel throughout Northern Virginia, Southern Maryland and DC to be able to cover our division.
REQUIREMENTS:
Current RN license in the State of practice.
Bachelor’s degree preferred.
Minimum of three years of nephrology experience in a PD/HHD setting.
CNN or CPDN, Bonent certification within two years of employment preferred
Current driver’s license in the state of residence and insurable driving record required..
