This is a Full Time temporary 1 year contract position

QUALIFICATIONS:

Registered Practical Nurse in good standing OR
Social Worker in good standing Registered with the Ontario College of Social Workers and Social Service Workers
· Current CPR and First Aid Certificates
· Training in P.I.E.C.E.S. TM and Gentle Persuasive Approach (GPA) · Familiarity With Positive Approach to CareTM (Teepa Snow)
The Manager of Cognitive Care (MoCC) oversees all care services that pertain to the mental health and wellness of the residents of Trinity Village Care Centre. This role is the lead for the Behaviour Support Ontario program and is a key point of contact for residents with predictable, responsive behaviours. The role is involved in performing interventions directly with residents, as well as working with the staff and families for appropriate follow-through and education regarding best practice in mental health. The MoCC advocates for residents by coaching and mentoring staff on methods to assist with responsive behaviours. This role is a department manager for the Nursing Department, and directly oversees the BSO PSW and Social Worker. The MoCC also works closely with external specialized services.

Responsibilities:

· Oversee all care services that pertain to the mental health and wellness of the residents by creating appropriate policies and procedures in alignment with best care practices, and the rest of the Nursing Department.
· Receive BSO referrals from registered staff, RAI MDS, and physiotherapist in order to evaluate and assess if they are appropriate for BSO services
· Collaborate with frontline employees to obtain the most current condition and patterns of residents in order to assess the source of behaviours and work together with them to determine appropriate personalized approaches
· To implement best practices in treating dementia, delirium and mental health issues, and awareness of the effect of these issues have on persons, families and caregivers.
· Protection of individual’s rights and advocacy role for the residents.
· Lead and coach staff in a variety of behavioural interventions, care approaches, and skills that will help with their interactions with residents
· Adhere to Trinity Village/LHKW’s Mission, Vision, and Values, and follow all organizational policies and procedures
· Adhere to the following initiatives for this role: Eden Alternative Philosophy of Care, Green Environmental Initiatives, and CARF Accreditation standards
· Choose attitudes and behaviours that are aligned with the Code of Conduct and Ethics, and ensure the health and safety of yourself and others at all times
· Actively participate in team meetings, organizational objectives, committees, and special events
· Remain compliant to all legislative bodies as required by Trinity Village/LHKW, and report as required
· Be familiar with and adhere to the Resident’s Bill of Rights as dictated by the Long-Term Care Homes Act
· Complete all mandatory training as assigned
· Maintain professional standards and certification as required for the role of BSO Lead through continuing education and professional development
