Description:

JOB OVERVIEW:

The Manager of Population Health, is responsible for the successful operation of activities of major significance to the organization. The candidate will lead programs, initiatives, and projects; and will be performing functions that enable the success of key business processes and practices. This position offers exposure to several high functioning departments, ambulatory groups, joint-ventures, and community partners looking to monitor and improve quality performance for their MSSP, Commercial, and Managed Care populations.

Responsibilities:

ESSENTIAL FUNCTIONS:

  • Lead and develop a team of professionals responsible for the quality improvement activities of our Ambulatory Groups, Joint Ventures, and Clinically Integrated Network and its performance in our various value-based programs with different Payors and CMS.
  • Lead the improvement of all the participating pay-for-performance programs, build workflow and deploy resources accordingly.
  • Manage the daily operations of the team and ensure schedules and performance standards are realistically set and attained.
  • Partner with other department lead to establish business goals for department/function/organizational unit and monitor status against goals.
  • Partner with Practice Optimization and Clinical team to ensure the practices are well-coordinated in total care.
  • Act as the liaison between Payors and Health System in various Quality VBP and Pay-for-Performance Program.
  • Conduct practice education on various quality measures for both Gov t and Commercial programs. Have broad understanding of supplemental data submission.
  • Lead multiple medical record retrieval project throughout the year, to close gaps in care, improve risk scores, and STAR ratings.
  • Assist with other various programs as it gets introduced through the Population Health Department.
