Langs was established as a community development project in 1978 by a group of citizens and service providers concerned about the lack of accessible services in the community. Langs has grown to become a dynamic, respected multi-service organization that uses a community development approach to provide comprehensive health, social and recreational services that are responsive to the changing needs of our community.
Responsibilities:
A SUCCESSFUL CANDIDATE IN THIS ROLE WOULD HAVE
· Relevant university level experience and/or training in health and leadership. A Graduate degree or additional relevant training would be an asset.
· Three or more years management experience in a multi-disciplinary clinical environment. Experience in a community-based primary health care setting is preferred.
· Three or more years in Community Outreach Experience an asset
· Broad knowledge in the areas of community-based primary health care, the social determinants of health and health promotion.
· Demonstrated leadership abilities within an interdisciplinary, culturally diverse staff team.
· Knowledge and experience in quality improvement processes.
· Demonstrated ability to work collaboratively.
· Strong organizational, administrative and time management skills.
· Effective decision-making and problem-solving skills.
· Excellent communication and conflict-resolution skills.
· Demonstrated ability to communicate effectively in English, both written and verbally.
· Ability to maintain professionalism and strict confidentiality as a member of the leadership team.
· Experience working effectively with culturally and economically diverse populations.
· Competency in Microsoft Office Suite. Proficiency with electronic health records systems.
· Valid driver’s license, access to a reliable personal vehicle
IN THIS ROLE YOU WILL BE EXPECTED TO COMPLETE THE FOLLOWING TASKS
· Coordinate and oversee the Outreach and Health Guide service delivery to ensure effective support for individuals and communities.
· Collaborate with the Clinical Director to manage the operational setup and responsiveness of the HART Hub to community needs.
· Support the development and implementation of an annual operational plan for Outreach locations.
· Lead and manage staff responsible for Outreach programming, including coaching, supervising, and supporting staff development.
· Establish and maintain strong partnerships with external agencies and community organizations to advance the mission of the HART Hub and Outreach services.
· Develop and implement communication strategies to ensure clear, effective communication across teams.
· Identify and apply for grant opportunities to support program growth and sustainability.
· Monitor workload and performance indicators, ensuring alignment with funder expectations and organizational goals.
· Coordinate and implement quality assurance activities, including chart audits, to support continuous improvement.
· Review and approve expenditures to ensure alignment with budgetary guidelines.
· Support the Director of Finance in developing and monitoring the operating budget, ensuring fiscal responsibility.
· Participate in leadership meetings, contributing to strategic planning and decision-making.
· Support the development and implementation of quality improvement initiatives for Outreach and Health Guide programs and services.
· Lead recruitment, hiring, and performance management processes for Outreach and Health Guide staff.
· Provide leadership in the development and implementation of quality improvement initiatives related to Outreach and Health Guide programs and services.
· Participate in regional planning meetings and contribute to the continuous evolution of services.
· Review and analyze service delivery data and provide insights to leadership to support decision-making.
