POSITION OVERVIEW

Within the Winnipeg Health Region there are over 5,700 people who live and receive care and health services in long-term care facilities.
The Long-Term Care program is an interdisciplinary team of individuals who work with Long Term Care facilities to provide a continuum of clinical services designed to support the needs of people living with chronic health problems that affect their ability to perform daily activity. Although caring for the elderly is the main area of focus the program provides care and services for individuals across the entire age spectrum.
The program provides content expertise, evidence informed recommendations/guidelines, and initiatives designed to sustain and improve the quality and consistency of care provided throughout Winnipeg’s 39 long-term care facilities.

Program purpose:

  • To advance the delivery of care and services in LTC through innovation, research, public engagement and collaboration among our partners and service providers.
  • To coordinate, support and ensure consistency in the delivery of high-quality long-term care services in a dignified, respectful and equitable manner to those requiring the service across all PCHs located within the WRHA.
  • To support Winnipeg PCHs in the planning, delivery, monitoring and evaluating of safe, high quality, efficient, and effective Long-Term Care services.
  • To assist, support and inform members of the public about the access to LTC including demand, capacity and applicable wait times with the ultimate goal of enhancing the resident and caregiver experience.

Reporting to the Executive Director of the WRHA Long Term Care (LTC) Program, the Transitional Care Manager will be responsible for the monitoring of care and operations within the Transitional Care Environment (TCE), addressing client relation issues, supporting the site with day to day operations and in meeting the outlined deliverables. The Manager acts as liaison between the WRHA and the TCE proponent. All duties are performed in accordance with WRHA related policies, established Health and Safety regulations/guidelines, and policies and procedures (e.g. utilizing personal, protective equipment as per safe work procedures). The Manager also provides support to LTC patient flow initiatives.

EXPERIENCE

  • Experience in program planning, development, evaluation and managing patient care services required.
  • Expertise in long term care preferred.

EDUCATION (DEGREE/DIPLOMA/CERTIFICATE)

  • Bachelor degree in Nursing required.

QUALIFICATIONS AND SKILLS

  • Demonstrated proficiency with the use of Microsoft Office products (Word, Excel, PowerPoint) required.
  • Knowledge of regional policies, procedures, protocols and guidelines.
  • Knowledge of the Personal Health Information Act, Workplace Hazardous Material Information Systems (WHMIS), Principles of Routine Practices, and other legislated acts.
  • Demonstrates effective communication and presentation skills and ability to communicate effectively both in oral and written form.
  • Ability to interact and lead effectively as a member of interdisciplinary team.
  • Problem-solves, organizes results and monitors overall operations of the Transitional Care Environment.
  • Prioritizes workload, responds to simultaneous demands and plans, manages, and implements change.
  • Demonstrates interpersonal, critical thinking, conflict resolution and decision-making skills.
  • Ability to function independently under minimal supervision.

Responsibilities:

THIS POSITION REQUIRES A CURRENT SATISFACTORY CRIMINAL RECORDS CHECK (INCLUDING VULNERABLE SECTOR SEARCH), CHILD ABUSE REGISTRY CHECK AND ADULT ABUSE REGISTRY CHECK AS CONDITIONS OF EMPLOYMENT. THE SUCCESSFUL CANDIDATE WILL BE RESPONSIBLE FOR ANY SERVICE CHARGES INCURRED. A SECURITY CHECK IS CONSIDERED CURRENT IF IT WAS OBTAINED NO MORE THAN SIX (6) MONTHS PRIOR TO THE START OF EMPLOYMENT.
