Description:

This is a really exciting opportunity to join the Manchester care group as a senior integrated discharge practitioner. This role will work as part of the inpatient and wider Manchester leadership team and the wider system.
The Manchester Care Group have developed a multi-disciplinary hospital based integrated discharge team to support the safe and rapid discharge of service users who longer require a hospital setting. The remit of the service will focus on service users from all adult acute, later life and PICU beds across the GMMH Manchester care group footprint, contracted beds within the North West Bed Bureau (NWBB) and out of area placements (OAPs).

The integrated discharge team will ensure the facilitation of timely discharge from the inpatient units across the locality and wider GMMH foot print, across the NWBB and OAPs to reduce wasted bed days in a hospital setting for Manchester locality patients. The post holder will lead on facilitating the discharge of complex cases and develop excellent working relationships with external partners to progress barriers in relation to service users deemed clinically ready for discharge.

  • Ensuring discharge planning from point of admission and ensure early identification of potential barriers.
  • Supporting teams with a positive risk based approach to discharge to reduce harm associated with long lengths of stay.
  • Utilising extensive experience of statutory assessments to ensure that they are of the highest standard.
  • Act as a ‘Trusted assessor’ in order to ensure Care act assessments are completed and are of a high quality in order to reduce delays.
  • Working with the most complex and lengthy delays including CRFD, progressing cases forward, utilising a multi partner approach to finding long term, sustainable community-based solutions
  • Work closely and collaboratively alongside clinical and operational partners across all services as required, focused on the timely resolution of internal and external barriers to facilitate discharge and maintain flow across the GMMH footprint.
  • Provide operational resilience support affecting flow, working closely with care groups to support the development, implementation, management and review of the Trusts operational plans, policies and escalation protocols which support the management of Patient Flow and discharge.
  • Case manage the most complex discharge cases, and support patient flow working with all key partners, whilst using their expert knowledge of community services to create seamless pathway of care from hospital to community and home.

Pro actively utilises own judgement to provide options and resolutions which may involve complex facts. Optomising all professional advice from the MDT, family, carers to drive a way forward for example: case conferences, vulnerable adult panels, guardianship applications
Oversee the daily delivery of discharges including complex cases and patients clinically ready for discharge.
To provide effective leadership for the Manchester Patient Flow Service spoke practitioners, promoting a positive and therapeutic culture.
Support the Care group at internal and external patient flow forums, sharing information, supporting improvement plans and taking issues forward.
Participate in local and regional networks to promote Manchester care group and patient flow
Retain daily oversight of discharge progress with live and accurate updates to the Head of operations, Associate directors and Strategic lead as required.
