• Schedule: 8am-5pm, Monday-Friday, Full Time
• Department: Skilled Nursing, 4D
• Location: Presbyterian Medical Center- Charlotte, NC
• Responsibilities Facilitates the timely and accurate completion of the minimum data set; certifies the need for appropriate care with awareness of costs, length of stay, and determines when level of care changes; insures successful transmission of data to the State.
• Unit Profile: Designed to cater to the unique needs of individuals with advanced dementia, delirium, depression, and those experiencing a decline in functional abilities or overall health. The staff in 4D skilled nursing units receive specialized training to provide comprehensive care and support to these residents, helping them manage their medical conditions, maintain their quality of life, and maximize their independence as much as possible.
