Description:
The opportunity has arisen for a Band 4 MDT (Multidisciplinary Team) Coordinator for the Thames Valley Vascular Network, based at Oxford University Hospitals NHS Foundation Trust.
The MDT Coordinator will ensure all patients are discussed at the weekly Vascular MDT meetings at the John Radcliffe Hospital, monthly Venous, Complex Aortic and Carotid Body Tumour MDTs by organising the patient lists and ensuring that all relevant clinical documentation is available for clinicians to make informed treatment decisions using agreed templates and processes. This role is essential for coordinating the delivery of care to ensure interventions are carried out as timely as possible, including carotid endarterectomy and abdominal aortic aneurysm repairs, as set out in the national service specification for specialised vascular services. The post-holder will operate as a main point of contact for clinicians and administrators across the network for responding to queries regarding a patient’s clinical investigations and treatment plan.
The role is administrative and will require great organisational skills and attention to detail. Excellent communication skills is essential as you will be working as part a large team. The role will involve liaising with both clinical and support staff. You will receive initial training for the role but knowledge of the NHS, NHS systems and previous experience of arranging and undertaking MDT meetings would be a significant advantage. You will need to be able to cope well under pressure, use your initiative to work individually and work well as part of a team.

MDT Management & Patient Pathway Coordination
Prepare and circulate lists of patients for discussion at each multi-disciplinary team meeting (MDT) using information provided by relevant parties in a timely manner.
To be responsible for ensuring the accurate and complete collection and management of the minimum data set for all patients to be discussed at MDT and to ensure all patient pathways are correctly mapped and appropriately tracked.
To attend the MDT meetings, keeping comprehensive records of attendance and outcomes of the meeting, and distributing to relevant team members in a timely manner.
Network Lead for the National Vascular Database to ensure information is accurately recorded and audited, and provide training on NVR data entry to staff members across the network.
To ensure that all operational changes affecting the MDT are communicated effectively to all members of the MDT and other interested parties
To check with the MDT Lead Clinician that all the information is correct and complete. To acquire Lead Clinician sign-off of all MDT outcomes.
Collection and interpretation of patient information from notes and MDTs in order to complete data.
Maintain filing systems for MDT’s enabling the team to easily and efficiently access core information
To act as the patient navigator and follow the patient journey from referral, through diagnosis, to treatment.
To utilise negotiation skills effectively regarding theatre bookings, diagnostic testing and any other relevant procedures regarding the patient’s effective pathway.
To ensure all information relating to the patient’s pathway clearly identifies all relevant target dates to aid other members of the MDT in allocating resources appropriately.
To identify bottlenecks/delays in the patient pathway and to follow the Trust escalation policy to raise awareness to the Service Manager as appropriate.
Be a point of contact for general enquiries regarding Vascular Surgery Waiting Times, MDT Co-ordination and Peer Review, providing expert advice and guidance to internal and external stakeholders.
Develop, maintain and audit the Thames Valley Vascular Network AAA Database and assist in data generation for research.
To provide basic analysis of the data for clinicians and the Service Manager and carry out routine audits of patient outcomes.
Take initial responsibility for local resolution of problems that arise among MDT patient pathway, taking the necessary actions to resolve the problems or referring to the appropriate person.
Data Management
To ensure accurate record of the outcomes of the MDT meetings.
Contribute to the Peer Review process by providing required audit data (e.g. NVR data, MDT attendance data)
To prepare for and highlight any long waiters for the weekly Inpatients PTL review meetings with the Assistant Service Manager.
Systems Management
To manage systems that informs referrers and clinical colleagues of patient’s treatment plans and decisions taken at MDT meetings.
To support the Patient Pathway Team Leader in ensuring that clinic templates are accurate and support in resolving data quality issues.
To develop specific expertise in all relevant Cerner applications (including Appointment Book, Powerchart, and PM Office), providing a source of expertise for all staff across the Thames Valley Vascular Network.
Service Improvement
To develop and instigate MDT service improvement projects.
To proactively identify issues with MDT co-ordination and outpatient clinic templates and escalate to Service Manager as required.
Assist with process mapping or any other methods of problem solving, as required for service improvement, and make appropriate recommendations for use of resources ensuring the organisation delivers quality service.
Communication
To communicate daily with a wide range of staff within the NHS. The post holder must be able to communicate with Consultants, Service Managers, Nursing Staff, Ward Clerks and Medical Secretaries.
To identify and discuss with the MDT Lead Consultant any MDT issues such as attendance records at meetings, problems with patients medical record collection and any service improvement issues.
To provide detailed patient information where required and to raise any information that is not discussed during the MDT meeting.
To attend Thames Valley Vascular Network meetings as and when required and present any service improvements or audits that have been completed.
To distribute the MDT list and treatment records following the MDT and discuss with the MDT Lead Consultant any missing records.
To contact patients, where appropriate, to explain the process for individual treatment pathways. Liaising with patients in regards to scheduled appointment, or general queries relating to their care pathway.
To escalate where appropriate any issues to the Network Manager, Service Manager and MDT Lead Consultant.
To be an expert in the operation of all MDTs, providing a single source of expertise and contact for all internal and external colleagues.
To regularly update the Vascular Clinical Lead and Service Manager on the status of all MDTs and proactively escalate any issues requiring senior management intervention.
Bookings
To book elective admissions, pre-operative assessments, diagnostics and post-operative clinics and investigations for patients when required.
Other duties
Assist in project work and in producing ad hoc reports.
To identify own training and development needs and undertake appropriate training/education as required.
Undertake any other duties requested as appropriate to the banding
