Description:

This is an exciting opportunity to develop and support the flow and discharge processes across the Medical Division.
We are looking to appoint a motivated individual for a 12 month fixed term period to work with our clinical site, ED and ward teams to support good flow and discharge processes. The post holder will also need to work with the other divisions to ensure good communication around flow and discharge and agree processes for escalation and support.
The post holder will also be responsible for the day to day management of our Discharge Lounge; to ensure that the team have processes that fully support the flow and discharge of patients from the wards.
The post holder will have a key role in shaping the organisation and management of flow processes across the division.

The post is initially for 12 months and would be an ideal secondment opportunity.

  • This post holder will be part of the Divisional Team, acting as a senior member of the clinical team that facilitates patients through Medical Services from decision to admit to discharge at Wye Valley NHS Trust
  • The post holder will act as an expert clinical leader that utilises competent professional knowledge and skills to appropriately assess, advise, coordinate, plan and undertake alignment of resources, activation of escalation and mitigation actions in relation to ensuring quality driven safe efficient patient flow.

To be accountable and responsible for the standards of care provided in the Discharge Lounge and advocate/maximise utilisation of the Discharge Lounge environment.

As a senior clinical manager and decision maker ensure that systems are in place to facilitate the smooth and safe transfer and discharge of patients through the hospital
As appropriate will assertively challenge any decision to acute admission/delayed discharge where a safe and effective alternative service can be provided that meet the need of the patient / service user
To act as a senior clinical manager ensuring all aspects of managing relevant service areas of the hospital site and its resources are being managed appropriately and safely in line with clinical governance and escalation management policies.
To expertly assess clinical acuity taking appropriate action during the management of patient flow and in handling emergency situations as a call taker and on site clinician.
Work closely with the site team and Surgical flow co-ordinator to manage flow through the hospital.
To provide expert knowledge to deliver a high standard of professional decision making in relation to the provision of urgent care services and management of patient flow across multiple services.
Acts as a senior professional in support of representing patient flow services when working with all members of the multidisciplinary teams internal and external to the organisation.
Will act to take immediate mitigation or performance actions where there are failures to meet expected standards in the delivery of patient flow services.
Act as a senior clinical manager responsible for the day to day management of the Discharge Lounge.
Responsibility to oversee that the coordinating and patient flow support staff are following protocols, SOPs and operational policy to inform their practices.
To undertake any other duties, required by line manager appropriate to the grade and relevant to the post
