Responsibilities:

  • Promote and facilitate communication to assist in the rehabilitation process
  • Maintain a strong clinical practice with excellent patient outcomes
  • Remain an active participant in the referral process, including participating in bi-weekly meetings with key stakeholders and facilitating referrals and intakes for new clients.
  • Ensure all services delivered align with best practice and safety protocols.
  • Act as the main point of contact for all clients in the MMP program, managing any escalations and regular safety check-ins.
  • Develop and nurture relationships with stakeholders and organizations that refer patients to CBI Health
  • Provide clinic input into the service delivery model and developmental opportunities
  • Ensure that the service delivered aligns with key performance indicators (KPIs)
  • Align local objectives with corporate and functional goals, identifying business metrics to meet and exceed CBI Health’s vision and values
  • Organize resources to solve problems and make tough decisions, both independently and as part of a team
