DESIGNATION : Medical Social Worker (Jurong West Nursing Home)

QUALIFICATIONS

• Degree in Social Work or related discipline
• At least 5 years of relevant experience
• Social work in healthcare background preferred
• Good interpersonal and problem-solving skills
• Caring, positive and self-disciplined individual
• Ability to work well in a team
• Proficient in Google Workspace (i.e. Gmail, Google Docs, Sheets, Slides etc)

Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents’ medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.
