Description:

To promote and support the safe, effective, high-quality, and cost-efficient prescribing and management of medications within the base locality and across Greater Manchester. This includes providing technical assistance in GP practices to implement and sustain improvements in prescribing practices, minimising medicines-related risks to patients.
The role involves acting as a medicines optimisation link for specified GP practices, implementing initiatives aligned with primary care and medicines optimisation priorities. Responsibilities include conducting prescribing audits, adhering to Standard Operating Procedures (SOPs), and assisting in the development of repeat prescribing systems under the direction of the line manager.
Additionally, the role involves managing Medicines Information inquiries within GP practices and social care settings and using prescribing data and technology to manage budgets and support Medicines Optimisation projects. The position requires collaborative work across NHS Greater Manchester or designated localities to drive joint initiatives and raise awareness of Medicines Optimisation issues.
Medicines Optimisation : Deliver Medicines Optimisation (MO) and primary care quality objectives, including QIPP initiatives. Improve prescribing habits in primary and secondary care.
Strategic Support : Collaborate with GM MO teams to implement programmes and support GP practices in meeting targets.
Collaboration : Build relationships with stakeholders (e.g., PCNs, pharmacies, LA) to implement workstreams and promote cost-effective prescribing.
Stakeholder Engagement : Support key agendas, including Quality, Safety, and Public Health. Enhance MO in care homes and for patients at home.
Advisory Support : Provide clinical advice to teams and address medication issues with GP practices, care homes, and pharmacies.
Data Analysis : Produce and interpret prescribing data to monitor progress, target support, and recommend actions.
Audits and Policies : Conduct audits to improve prescribing quality and develop SOPs, policies, and guidelines.
Training : Deliver training to healthcare staff and share prescribing updates during meetings and events.
Patient Support : Perform MO reviews in care settings, addressing risks for vulnerable patients.
Technical Tools : Maintain prescribing support tools and analyse software reports for cost-effective practices.
Workload Management : Manage tasks with minimal supervision, recognising professional limits.
This role enhances prescribing quality, reduces medicine-related risks, and improves patient safety across care settings.
